A Yearly physical is required by Summit Pediatrics to fill out any forms ie, Sports Physicals, Daycare, Working Papers, Asthma forms etc..

The yearly physical allows us to monitor your child’s growth and development, discuss a healthy lifestyle and ensure up to date medical care and immunizations.
We utilize electronic medical records, linking all 3 of our locations.  This also allows us to
access hospital, lab and pharmacies instantly.   We are excited at the opening our
Patient Portal which is designed to enhance patient-physician communication.
We have created this site to provide our patients and families with 24 hour access to
information about our practice, some answers to commonly asked questions and some useful,
reliable health links.
At Summit Pediatrics, you are never alone.  We are always here, at your service, day and

At Summit Pediatrics we are committed to
vaccinate all children as per the
recommendation of A.A.P./CDC.   In preserving
the best interest and safety of our patients,
especially now with the recent outbreaks of
Measles and Pertussis ,  we ask parents to
please review our Vaccine Policy and to
understand that we will no longer  accept
patients in our practice that cannot agree with
this policy.

In our 2015 Patient Satisfaction Survey Summit Peds received a score of 100% in Accessibility
provided for our families. Summit Peds believes that it is important to our patients to receive care
when they need it. With our Electronic Medical Records your charts can be accessed at any of our
locations. We encourage continuity of care and when possible will schedule your appointment with
the Provider of your choice no matter where he/she is located.

We are proud to make known our commitment to our patient’s Health and Safety.
Our Care coordinators have been working with our patients that have Chronic conditions such as
Asthma, ADHD, and unhealthy weight.

Since obtaining Patient Centered Medical Home recognition through the
National Committee for Quality Assurance,

our providers and care coordinators have kept 96% of our Asthma patients current with their
Asthma Action Plans, helping them understand their medications, their proper use, and using their
self care management plans.
This year we have started to work with families of our littlest patients, premature babies, to make
sure they get the necessary developmental screenings, providing  community resources, and to
help them overcome any barriers that may interfere with their childs growth and development.
Please feel free to let us know how we're doing, whether good or bad. Input from our families is
very imprtant to us. No detail is too small!!
